   • Home Address
   • Proof of age
   • Proof of Income

2. Pick a date that works for you

   Our open houses operate on a first come, first serve basis for your convenience.

   See below for our open house times and locations.

3. Meet with a Facilitated Enroller

   This is the person who will assist you in the application process.

   They are located at all of the sites during the times listed.
